You will get your Member ID card in the mail soon. CareOregon picks a main doctor for you, called a primary care provider (PCP). Their name is on your Member ID.
When you go to your doctor or the pharmacy, bring these two things:
- Your Health Share Member ID card
- A photo ID (like a driver’s license)
Welcome call from CareOregon
If you are a new member, someone from CareOregon will call you soon. This call is to:
- Make sure your information is right
- Tell you about your health benefits
- Help you find the care and services you need
We will be happy to answer any questions you have about your coverage and benefits.
Take the Health Risk Assessment survey
We will also send you a Health Risk Assessment (HRA) survey in the mail soon. It's a short survey. Please fill it out and send it back. It helps us understand your health needs and how to support you even more.
Schedule a wellness visit with your PCP
As soon as you can, call the PCP listed on the front of your Member ID card. Tell them:
- You're a new patient
- You're a CareOregon member
- You want to set up a wellness visit (a check-up)
If you take any medications, ask your doctor if your health plan covers it. You can also check the drug list.

If you want to choose your own doctor, Customer Service can help. You can check if a certain doctor is in our network. Or you can find a different doctor who is taking new patients by using our Find a provider tool.
Log into the member portal or mobile app
You can log into the CareOregon member portal or app to:
- See your health benefits
- Look at your prescriptions
- Send us a secure message
- Find our contact information
- See your referral history

The Health Share of Oregon Member Handbook tells you about your health plan. It includes:
- What services are covered
- How to get care
- What to do if you have questions
You can download the latest handbook below or call to ask for a paper copy by mail.
Stay connected to care
To keep your Oregon Health Plan (OHP), you may need to reapply each year. OHP will send you a letter about how to do this.
If your name, address, or phone number changes, tell OHP. Also, if you are pregnant or just had a baby, tell OHP right away.
