How to qualify for Tribal Care Coordination

CareOregon’s Tribal Care Coordination program is for anyone who identifies as American Indian or Alaska Native and has the fee-for-service (open card) Oregon Health Plan. You can even join if:

  • You are not officially enrolled in a tribe
  • Your tribe is not federally recognized
  • Your tribe is from outside Oregon 

This program also includes members of Oregon’s nine federally recognized Tribes (see full list below) and NARA (Native American Rehabilitation Association).

We work together with Tribal leaders, Tribal health programs and NARA to give you extra help with your health care needs.

There is no cost for this service, and you can leave at any time.

What is Tribal Care Coordination?

Our trained CareOregon staff can connect you to programs that keep you healthy. We can help link your doctors, other providers, local support agencies and social service organizations. Our goal is to work with your care team, answer your questions and help you get past any challenges to your health.

The program can help you:

  • Find providers, including for mental health and substance use treatment
  • Get appointments with primary care providers, specialists and dental providers
  • Get rides to medical appointments
  • Get medical equipment, supplies or other needed services

Care management

If you have a serious or long-term health condition, our care team can help you.

This includes conditions like:

  • Tiểu đường
  • Heart failure

  • Substance use (like drugs, alcohol or tobacco)
  • And more

Our team includes a:

  • Nurse
  • Mental health specialist
  • Health care coordinator

Together, they can help you understand your treatment plan set by your primary care provider (PCP), make the most of provider visits and connect you to more help in your community.

Care management teams also talk with you about how to live well and feel better with your condition. We want to help you feel positive and supported.

Intensive care management

If you have more than one chronic condition, special health care needs or a disability, you may qualify for intensive care management services.

Your PCP will usually decide if you need these services. But you also can contact us if you think they would help you.
