CareOregon’s Tribal Care Coordination program is for all self-identified American Indians and Alaska Natives on the fee-for-service (open card) Oregon Health Plan. These members may be not formally enrolled in their tribes or their tribes may not be federally recognized, and/or their tribes may not be from Oregon region. This program also includes members of Oregon’s nine sovereign, federally recognized Tribes (see full list below) and NARA (Native American Rehabilitation Association). We work together with Tribal leaders, Tribal health programs and centers, and NARA to offer extra help with your health care needs.

What’s in this program?
Our trained staff at CareOregon can connect you to programs that help you stay healthy. We can help link your doctors, other providers, local support agencies and social service organizations. Our specialty is to work together with your care team to help answer any questions you have and help you overcome barriers to your health.
The program also can help you:
- Find a provider for your health care needs, including mental health and substance use treatment.
- Get appointments with primary care providers, specialists and dental providers.
- Get transportation to appointments.
- Get needed medical equipment, supplies or services.
Care management
If you have a serious or chronic health condition or disease, we have a team of professionals who are ready to help you.
This includes conditions such as:

Our team includes a nurse, mental health specialist and health care coordinator. Together, we can help you understand treatment plans set by your primary care provider (PCP). We will also help you make the most of provider visits and connect you with more help in your community.
Care management teams talk with you about how to live well and feel better with your condition. We want to help you feel positive about life.
Intensive care management
If you have multiple chronic conditions, special health care needs or if you are disabled, you may be a candidate for intensive care management services. Your primary care provider (PCP) will most likely determine this, but you also can contact us if you think you need these services.
